Any factual error inRegulated to Fail — a figure, a date, a source, a characterisation of what an organisation did — is recorded here, with the original wording alongside the correction, so that a reader holding a printed copy can see exactly what changed.

Corrections are listed separately from updates. An update is something that happened after the evidential cut-off of 14 August 2026. A correction is something this book got wrong.
